PE Exemption
This form is used for high school students to be an exemption from PE if they are in a full schedule.  

Community Service
We no longer require hours to graduate, but honor 100 honors for the gold cord award. 

College Visit
Students need to take the college visit form to the college visit and get the college official to sign to excuse them for the school day. 

Student/Counselor Referral
If your students need to see a counselor for various reasons complete this form. 

TRIO Application
This program is not students planning on attending college, but are first 
generation college students,  low income, and need assistance with college documents. 

Connect to College Application
This application needs completed with a students social security number in order to take any college courses for dual credit.

Work Based Learning Experience
If you are working part time and would like to use your work house as a credit opportunity or would like on the job experience please complete this form In order to get high school credit.  

Mentor Program
Mentors will complete the application to mentor peers. Upon completion of reference checks and permission from parents. The School Counselor will then match the mentor with a student in need. Students in need will be referred by staff members, parents, guardians, coaches. Mentors will meet with the mentee weekly. Students must complete 60 hours/meetings with their mentee to obtain 1 credit for high school. Mentors must be confidential, respectful, kind, helpful, honest, and responsible. Mentors will check in with the School Counselor monthly to review documentation and participate in coaching sessions.

Mentor/Mentee Referral Form 

Small Group Referral   This is a referral form to get students into small counseling and social skills groups to work on skill development.
